Xbox 360 - Gears of War 2 and Halo 3?

I'm planning on playing the campaigns for Gears of War 2 and Halo 3 soon. I have the same question for both.

Gears 2 - If you beat it on insane do you get the achievements for casual, normal, hardcore, and insane or just insane? Do I really have to beat it 4 times?

Halo 3 - Same thing. If I beat the game on legendary, does it unlock the achievements for the other difficulty levels?

i dont play gears but when i beat halo 3 on legendary i only got the achievement for legendary and for beating the last level and whats the point of xbox without live?
