How can we track who accessed a document?

Sometimes files disappear or get altered on our shared drive, and nobody admits to doing it. How can we track exactly who accessed and modified each document?

In one of my previous roles, we struggled with the same mystery — files were vanishing or changing, but there was no audit trail. The fix came when we switched to a platform that automatically logged all activity: who opened the file, when, and what changes were made. It instantly made people more careful, and the number of “accidental” edits dropped sharply. I think it's possible to track or trace the IP address used by the person to access the documents especially when it was done online. It's going to be very difficult to do it when it's locally done on a computer.
