Sometimes files disappear or get altered on our shared drive, and nobody admits to doing it. How can we track exactly who accessed and modified each document?

I think it's possible to track or trace the IP address used by the person to access the documents especially when it was done online. It's going to be very difficult to do it when it's locally done on a computer.
